At your first appointment a thorough exam will be performed by Dr. Schabes that consists of the following: Full series of x-rays Oral exam Periodontal charting (gum measurements and exam) Oral cancer screening Smile analysis Please bring a list of any medications (and dosages) you are taking so the doctor can see if the medication will have any affect on your dental treatment. You can fill out our welcome and health history forms online or call our office and it can be mailed to you. If you choose the online forms, click PDF files below and print out.
